Opened in April of 2001 to serve a greater population base, this location offers a variety of rehabilitation services. Community Rehab - West Omaha is located in the Charlies' on the Lake building at 144th and F Street.

Phil Davidson, PT/ATC
Phil is a native of Stanton Nebraska. He received a Bachelor of Education degree from the University of Nebraska -Lincoln in Physical Science and Athletic Training in 1987. He received a Master of Education degree in Sports Medicine from Temple University in 1988. Phil earned his Physical Therapy degree in 1991 from Rockhurst University in Kansas City Missouri.
Phil worked for two years as the head athletic trainer at Adams State University in Alamosa Colorado and 2 years as the head athletic trainer at Rockhurst University in Kansas City Missouri. Phil worked in Eugene Oregon and Omaha Nebraska as a physical therapist prior to starting Community Rehab Physical Therapy.
Phil is one of the co-owners of Community Rehab Physical Therapy founded in 1998 and Community Home Health founded in 2006. Community Rehab Physical Therapy has nine clinical locations in the Omaha and Sioux City area with a focus is on outpatient musculoskeletal injury rehabilitation.
Phil is active in patient care and healthcare education. Phil is active in the American Physical Therapy Association and the National Athletic Trainers Association. He works with area outreach programs to educate the public on the benefits of physical therapy to the individual and to the community. |

Sundee Hill, DPT
A native of Morrill, NE, Sundee received her Bachelors of Science from University of Nebraska - Lincoln. She received her Doctorate of Physical Therapy from Creighton University in 1998. Sundee worked extensively with children with developmental disabilities prior to joining Community Rehab in 2001. While at Community, she has expanded her education taking courses focusing on treating the pediatric athlete, and has been trained in postural restoration for the neck and back. She is a member of the orthopedic and pediatric section of the American Physical Therapy Association. Sundee coordinates fundraising efforts of Community employees, supporting the Salvation Army Holiday drive, Big Brothers/Big Sisters of Omaha, and the Douglas County Relay for Life. |

Jacob Ulffers, DPT
Jacob is a native of Stromsburg Nebraska. He attended the University of Nebraska at Omaha for 3 years with a focus on exercise science. He was accepted into the University of Nebraska Medical Center Physical Therapy program in 2005, of which he graduated in May of 2008 with a Bachelors of Science in Medicine and a Doctorate of Physical Therapy. Jacob is a member of the American Physical Therapy Association Orthopedic Section and the Nebraska Physical Therapy Association. |

Jeremy Beranek, ATC
Jeremy grew up in Ashland, NE. He earned a Bachelor of Science Degree in Exercise Science with an emphasis in Athletic Training and Coaching from Wayne State College. Jeremy has worked at Community Rehab since 2003 and is a member of the National Athletic Trainers Association. He is the Athletic Trainer that handles the outreach services to area high schools, coordinates the durable medical equipment located at each clinic, and assists in the marketing of Community Rehab. Jeremy and his wife Joy have a son, Andrew. |
