Opened in April of 2001 to serve a greater population base, this location offers a variety of rehabilitation services. Community Rehab - West Omaha is located in the Charlies' on the Lake building at 144th and F Street.

Phil Davidson, PT/ATC
Phil is a native of Stanton, NE. He holds an undergraduate degree from the University of Nebraska-Lincoln and a master's degree from Temple University. Phil earned his physical therapy degree from Rockhurst College in Kansas City. Phil is one of the co-owners and founders of Community Rehab Physical Therapy & Sports Medicine. Phil worked for two years as the head athletic trainer at Adams State University in Alamosa, Colorado prior to practicing physical therapy. Phil worked in Eugene, Oregon and in Omaha prior to founding Community Rehab Therapy & Sports Medicine. He is an active member of the Nebraska Chapter of the American Physical Therapy Association. |

Sundee Hill, DPT
A native of Morrill, NE, Sundee received her Bachelors of Science from University of Nebraska - Lincoln. She received her Doctorate of Physical Therapy from Creighton University in 1998. Sundee worked extensively with children with developmental disabilities prior to joining Community Rehab in 2001. While at Community, she has expanded her education taking courses focusing on treating the pediatric athlete, and has been trained in postural restoration for the neck and back. She is a member of the orthopedic and pediatric section of the American Physical Therapy Association. Sundee coordinates fundraising efforts of Community employees, supporting the Salvation Army Holiday drive, Big Brothers/Big Sisters of Omaha, and the Douglas County Relay for Life. |

Ken Noecker, PTA
Ken is originally from Hartington, NE. He is one of 12 children. Ken graduated from the Clarkson PTA program in 1996. He has worked at Community Rehab since 1999, helping start up the Plattsmouth, LaVista, and Millard locations. Ken has focused his continuing education on shoulder and knee rehabilitation. He has significant experience with soft tissue mobilization, myofascial release, and massage technique. He is a member of the orthopedic section of the American Physical Therapy Association. Ken and his wife Leslie have three children, Grace, Ellie and Jack. |

Jeremy Beranek, ATC
Jeremy is originally from Ashland, NE. He earned a Bachelor of Science Degree in Exercise Science with an emphasis in Athletic Training and Coaching from Wayne State College. Jeremy joined Community Rehab in August of 2003. He is currently the Athletic Trainer for Plattsmouth, Ashland-Greenwood, Conestoga, Louisville, and Weeping Water High Schools. Jeremy is also a member of the National Athletic Trainers Association. |

Melanie Milledge
Melanie is originally from Council Bluffs, IA. She attended Creighton University and the University of Nebraska at Omaha. Melanie worked at the University of Nebraska Medical Center for 10 years prior to joining Community Rehab in January, 2006. Melanie is married and has at teenage stepdaughter. |
