Four months after starting in Fremont, Community Rehab began to offer services to the Omaha area with the opening of a LaVista clinic in May of 1998. The clinic relocated to its' current site in June of 2002 which it is still in the Baker's Square Plaza but just north of Office Depot on Northwest corner of 84th and Giles.

Pat Wilson,
PT/CSCS
Pat is from Shelby, IA and moved to Omaha where he did his undergraduate studies University of Nebraska - Omaha. He received his Masters in Physical Therapy from the University of Nebraska Medical Center in 1994. Pat is a Certified Strength and Conditioning Specialist and recently became an Orthopedic Certified Specialist in 2004. He joined Community Rehab Physical Therapy in May 2005. Pat is married to his wife of 9 years, Kari, and has four children Nick, Ben, Allie and Brandon. |

Sarah Heywood
Sarah grew up in the small town of North Bend NE and graduated from North Bend Central High School in 1997 and is currently living in Fremont NE. She joined Community Rehab Physical Therapy in September 2006 and is currently the office manager for the LaVista location. Sarah is also the proud mother of a son named Phoenix. |

Tanner Thomas, DPT
Tanner Thomas is a native of Omaha, Nebraska. He received his Doctorate of Physical Therapy degree from the University of Nebraska Medical Center. He joined the Community Rehab team in May of 2006. In his spare time Tanner enjoys spending time with his family, playing sports, hunting and watching the Atlanta Braves and Nebraska Cornhuskers. |

Tammi Murdock, PTA, LMT
Tammi Murdock is originally from Holdrege, Ne and graduated from Holdrege High School in 1998. She then went to Trinity International University in the Chicago area, where she majored in sport and wellness management for 2 ½ years. From there, Tammi went to Omaha School of Massage Therapy where she received her certificate of licensure. She has been a licensed massage therapist for 5+ years and currently works part-time at Creative Hair Design in Omaha, NE.
Tammi then went to Clarkson College and received her associates degree as a physical therapist assistant in August 2006. She has worked at various Community Rehab locations since her graduation. Tammi lives in Omaha with her husband and two step sons, ages 7&8. |
