Community Rehab's first clinic opened in Fremont in 1998. The clinic relocated to its' current site in May of 2003.

Mary Theiler, MPT
Mary’s hometown is Wood River, NE. After completing undergraduate studies at Doane College, she went on to graduate with a Masters in Physical Therapy from the University of Nebraska Medical Center in 1993. Mary, along with Phil Davidson, co-founded Community Rehab Physical Therapy & Sports Medicine, in 1998. Mary’s practice is based out of the Fremont clinic. She is a member of the American Physical Therapy Association.
Orthopedic rehabilitation has been the focus of her continuing education, with particular emphasis in shoulder, knee and lumbar injuries. Mary is also certified to fit orthotics through Sole Support, Inc. She enjoys working with a variety of patients, from the younger developing adolescent to the active elderly adult.
Mary and her husband Frank live in Arlington and have three children, Catie, Andy, and Sarah. |

Jamie Crowe, DPT, ATC
Jamie is originally from Gretna, NE. She received her Athletic Training education and degree in Biology/Psychology from Northwest Missouri State University in 2002. She earned her Doctorate of Physical Therapy from the University of Nebraska Medical Center in 2006.
Jamie started with Community Rehab in 2003 as a Certified Athletic Trainer, providing athletic training services to Arlington High School. In May 2006, she joined as a fulltime Physical Therapist.
Jamie is a member of the American Physical Therapy Association and National Athletic Trainer's Association, both state and national chapters. She has helped in organizing the SportsMetrics ACL Injury Prevention program for area athletes.
Jamie and her husband Adam live in Fremont and enjoy participating in community athletic and civic activities. |

Jamie Blum, DPT
Jamie is a native of Manning, Iowa. She earned her Bachelor of Science degree in 2005 from Iowa State University where she majored in biology and minored in psychology and coaching. In 2008, she received her Doctorate of Physical Therapy from the University of Nebraska Medical Center. She joined Community Rehab in February of 2009. Jamie is a member of the American Physical Therapy Association Orthopaedic Section, Nebraska Physical Therapy Association, and Omaha Sports Medicine Alliance. In her spare time Jamie enjoys spending time with her husband, Jeff, and their black lab, Mia.
