Community Rehab's first clinic opened in Fremont in 1998. The clinic relocated to its' current site in May of 2003.

Mary Theiler, MPT
Mary’s hometown is Wood River, NE. After completing undergraduate studies at Doane College, she went on to graduate with a Masters in Physical Therapy from the University of Nebraska Medical Center in 1993. Mary, along with Phil Davidson, co-founded Community Rehab Physical Therapy, in 1998. Mary’s practice is based out of the Fremont clinic. She is a member of the American Physical Therapy Association.
Orthopedic rehabilitation has been the focus of her continuing education, with particular emphasis in shoulder, knee and lumbar injuries. Mary is also certified to fit orthotics through Sole Support, Inc. She enjoys working with a variety of patients, from the younger developing adolescent to the active elderly adult.
Mary and her husband Frank live in Arlington and have three children, Catie, Andy, and Sarah. |

Jeff Foster, DPT
Jeff is a native of Valley, Nebraska. He received his bachelor degree in Psychology from the University of Nebraska-Lincoln in 2008. Following this, Jeff attended the University of Nebraska Medical Center in Omaha, Nebraska where he received his Doctor of Physical Therapy degree in 2011.
Jeff has had experience in a variety of areas including pre and post-surgical orthopedic care, neuromuscular rehabilitation, acute hospital care, and sports medicine rehabilitation. He has a particular interest in the areas of sports medicine as well as pre and post-operative care.
Jeff is an active member in the American Physical Therapy Association (APTA) and has attended continuing education courses covering a wide variety of topics. These areas include treatment and mobility for the bariatric patient, spinal related disorders, and various other orthopedic topics. He also has a special interest in continuing his education regarding advanced manual treatment in care related to the spine and back pain.
Jeff and his wife, Kristen, live in Omaha and enjoy participating in outdoor activities including fishing, hunting, and hiking. In his spare time Jeff enjoys being outdoors and watching high school, collegiate, and professional sports.

Emily Monahan, ATC
Emily is originally from Muscatine, Iowa. She attended Saint Louis University where she obtained a B.S in Exercise Science in 2010 and a Master's of Athletic Training in 2011. While at Saint Louis University, Emily was on the cheer and dance team for four years in addition to studying abroad at Bond University in Australia.
Through SLU's athletic training education program, Emily had the opportunity to work with all levels of athletics ranging from club, middle school, high school, collegiate, and semi professional athletics. Emily is an active member of the National Athletic Trainer's Association.
In her spare time she enjoys exercising, being outdoors with her family and friends, traveling, playing with her dog Charlie, and supporting the Iowa Hawkeyes!
